Below, is a section of the wording as it appears in our Your Functional Genomics User Consent to Use document that each patient/client agrees to before any raw data is uploaded into our software program. We promise never to sell the data or use it for research outside of our program, which is discussed in the following paragraphs as to how we use the data for research purposes. We would be more than happy to discuss this over the phone should you have additional questions or concerns. I understand that Functional Genomic Analysis™ agrees to keep all personal identifiable information completely confidential, and will not share it, or any health information with any other source, other than the health professional who ordered this test for me.
Other than creating the report for me and my health professional, I understand my genetic data is only used by Functional Genomic AnalysisTM to calculate population-based data within the software. The population data (which never includes any personal identifiable information) may be published and used for research purposes.
This data will be available to me in my gene report where I will be able to view the percentage of heterozygous and homozygous SNPs for the total population within the software.
I understand that my functional genetic information will only be disclosed to the clinician who ordered the test. I understand that my report will be available to me in my portal which will require me to log in to access. I understand that this portal is not linked to my electronic medical record and I do not have to have my full or real name in the portal if I do not want it there. I understand that I can speak with my clinician about what identifying information I want in the portal.
I understand that if I submit my saliva for DNA testing using the Your Functional Genomics DNA test kit, my DNA will be stored by the laboratory unless I specifically request that it be destroyed (the sample will only be identifiable by the bar code that was on the collection tube). If I wish for my sample to be destroyed, I acknowledge that I will be responsible for contacting Functional Genomic AnalysisTM directly with this request at contact@FGAnalysis.com. I also acknowledge that should I decide to pursue any further DNA testing, I will be financially responsible for obtaining a new test kit and resubmitting my sample should I decide to have my initial sample destroyed.
I understand that if I submit my saliva for DNA testing using the Your Functional Genomics DNA test, the stored DNA will be stored only for the express purpose of allowing future functional tests (i.e. ability to run further genomic tests from this same sample as future science evolves). I understand that if storage consent is granted it will be stored for as long as deemed necessary unless consent is withdrawn. I understand that DNA is stored only with a barcode as an identifier in order to protect privacy. I understand that personal information is never stored with the DNA sample. I understand that if I elect to have my DNA destroyed there will be an additional charge should I decide to update my report in the future.
This user agreement may be updated at the discretion of Functional Genomic AnalysisTM.
To rest your password:
1. Click on the “Forgot Password?” button on the homepage.
2. Then enter your Username and Email (which is typically the same).
3. After entering your Username and Email, click “SUBMIT”.
a. A message will appear letting you know that an Email has been sent.
b. You must leave this window open while you retrieve the Reset Key from the Email.
4. In the Reset Your Password Email, you will be provided a Reset Key. The Reset Key must be copied and pasted back into the window you left open.
5. Once you copy the reset key back into the window, you can then create a new password.
6. Once you have created a new password, click “Submit”. You will then be able to log back into your account with your new password.
NOTICE: As of December 2023, 23andMe is not allowing the raw data to be downloaded.
To download your 23andMe raw data: Your genome is ready when an email is received from 23andMe saying 'Your initial reports are ready'
*You must use a computer to download your data. No mobile devices.
- Go to 23andMe.com and log in to your account (if this is the first time you are logging in you may be required to navigate through several interactive screens)
- Click on the drop-down box next to your name
- Choose 'Browse Raw Data'
- Click ‘Download’
- Scroll down and click on the check mark "I understand the limitations and risks associated with uploading my information to third party sites." Then, choose 'Submit Request'
- It will say that your download is in progress and to expect an email. Typically, if you refresh the page in 3-5 minutes it will be available to you.
- Then, choose 'Download Raw Data'
- Save this file on your computer where you can easily locate it
- The file will download as a .zip or .txt file, this is what is needed to properly upload to the software program. DO NOT OPEN OR SAVE AS A PDF.

To request and set up an account:
- Use Google Chrome as your internet browser for optimal functionality. We recommend using a desktop computer, no phones or tablets when requesting an account.
- Visit our website at www.FunctionalGenomicAnalysis.com and click on the 'Practitioner 'graphic, then click on ‘Request Account’ from the home page.
- From there, fill out the application form with all the required information and click ‘Submit’ (the email you enter will be your Username).
- Once your account has been approved you will receive an email* with a link to Confirm Your Account. *Make sure to also check your spam folder for this email.
- Click ‘Confirm Your Account’ to complete setup.
- From there, fill out all required information and click ‘Next Step’. Passwords must be complex and consist of a capital letter, a lowercase letter, a symbol (a period does not count), and numbers (e.g. FGAmh&1234).
- Confirm Your Details, please ensure that all your information is entered correctly and click ‘FINISH’.
- You will then be taken to your Practitioner Portal page where you may add a client/patient and order a Your Functional Genomics DNA test kit*, or upload previously* received raw data from 23andMe and AncestryDNA. *We no longer recommend purchasing a 23andMe or AncestryDNA if you are wanting a nutritional assessment only.
